FEES AND PAYMENTS
PAYMENT:
- Payment is required at the time of your consultation booking.
- Payment methods: cash, check, Visa, Mastercard, Discover and American Express.
- HSA/FSA eligible expense.
CANCELLATION POLICY:
- Missed Appointments and/or canceled appointments without at least a 24-hours' notice, will be charged a $50 fee with the credit card on file.
ADDITIONAL FEES:
- Multiple babies (twins, triplets, ect) are an additional $50/baby and require a 1.5-hour initial consult.
VISITS
OFFICE VISITS:
- Office Location at Blossom Baby Ultrasound Studio: 2277 State Road Suite C2, Plymouth, MA 02360
- Office Hours are Mondays and Wednesdays
HOME VISITS:
- If you are using TLN and are out of my 15-mile radius, a $25.00 travel fee will apply.
VIRTUAL VISITS:
- Are not recommended or appropriate for all situations. Contact me before booking if you have any questions on your appointment.
- Are conducted via a HIPPA-compliant video chat program.
What you will receive after a consult:
AFTER YOUR CONSULT:
- An emailed personalized detailed Plan of Care within 24 hours of the consult.
- Unlimited email/phone/text access to me for any breastfeeding related questions.
INSURANCE:
- I can NOT guarantee that your insurance company will reimburse you for my services.
- I WILL provide you with an itemized receipt properly coded for your insurance company, after our visit.
